This is on the profile page. I'm hoping there's a way to make this set by default without having to modify the PHP templates. (fingers crossed)

Not clear exactly what you are wanting.
On what page are you looking at?

Sorry, that was in the extremely fuzzy category...

The Profile Settings for members. (Profile > Gear)
http://empathcommunityeliselebeaucom.jamroomhosting.com/profile/settings/profile_id=1/id=1

I would like all new members to have this box CHECKED by default.

Unfortunately that checkbox is hard coded to be off by default.
A simple custom listener module could be built to listen for new profiles being created then set that field 'on'.
I recall we've talked about something else custom. What was that? Maybe something can be done in time for you going live?

Maybe I am misunderstanding what this options does.

To prevent spamming, we want our members to approve their followers first (I call them friends) before they can receive messages from them.

If this box is not checked, does that mean that anyone can send messages to anyone, which is the default behavior?

In the Private Notes quota section, make sure that 'Followers Only' is checked. Then, 'followers' cannot send messages to 'followees', but 'followees' can message their 'followers'.
When setup like this, users cannot friend/follow anyone just to spam them. They can only message 'followees' if they have reciprocated.
Hope that makes sense.
