and also when you click next icon previous icon is updated as a question mark icon.

Clear your icon cache.
ACP -> CORE -> SYSTEM CORE -> TOOLS -> RESET CACHES -> Reset Icon Cache (CHECK) -> RESET SELECTED CACHES

After clearing the icon cache, make sure to clear your browser cache too.
