At the moment only one label is available for one chained select independent of the chain levels.

completed Chained Select Improvement
This is now supported in Chained Select 1.1.1 - in the "modify" sections for levels 2/3 you can set an independent label for the select option if you would like.

just tried here right now and did not have any issues.
Clicked on core update. It finished and when the page refreshed, it was on the 'System Updates' tab. I clicked on "Update All Items" and it all updated normally.
Are there any errors in your console. Try running the integrity check then come back to updates to see if that makes a difference. and check the "clear cache" checkbox while you run.
See how that goes.

I had this same issue and it was a packaging failure in the Core module for a few hours - I got it fixed up, but if you ran into this it means that jQuery failed to load.
- go in via FTP and delete the contents of the following directories:
- data/cache/jrCore/*
- data/cache/ActiveSkin/*
