If you do not have the Simple Custom Forms module installed and activated, then the form/contact_us URL will not work.

I've put a check in all the skins footer.tpl file to use the skins contact_us.tpl file if the custom forms module is not active.

It looks like this:

From the jrNova/footer.tpl file:

                        {if jrCore_module_is_active('jrCustomForm')}
                            <a href="{$jamroom_url}/form/contact_us">{jrCore_lang  skin=$_conf.jrCore_active_skin id="68" default="Contact Us"}</a>
                        {else}
                            <a href="{$jamroom_url}/form/contact_us">{jrCore_lang  skin=$_conf.jrCore_active_skin id="68" default="Contact Us"}</a>
                        {/if}

You'll also want to make sure you have the latest contact_us.tpl file.

I have been having the same concern. contacts_us leads to a page with no contact form, just this message:

Any tutorial on how to create a descent contact form in JR5? (ProJam Light here). I couldn't find the Simple Custom Forms module in my system.

The latest ProJamLight files should have a contact_us.tpl file with a contact form. And the contact us link (in the footer) has a check on it to see if the custom forms module is active, if it is, then it uses the form/contact_us URL, if is not active, then it uses just the contact_us URL which is the contact_us.tpl file.

Is ProJam Light version 1.0.8, the latest version? It is the one I am running. Still I can't figure out how to active the custom forms module. I don't even find it.

Please help.

Yes, that looks like it is the latest version, however... If you've made any modifications to your footer template via the template editor, the new code may have not been added.

You should see a Forms drop down section in the admin menu if you have the Custom Forms module installed. It is the same section where the Chained Select, TinyMCE Embed Button and TinyMCE Image Upload modules are. If you don't see it, then you don't have the module installed.

If you don't have the module installed, and your footer link doesn't take you to the new contact_us.tpl file, then your footer code probably hasn't been updated to the latest.

Try switching back to the default footer template and see if the link works then.
