Issue tracker missing an item

There seems to be something wrong with the issue tracker. One of my Feature Requests is not listed, and one is repeated twice.

I have 11 feature requests, the 11th (which is a completed request and is on Page 2) is a repeat of the 9th.

I can't see anything about the items in the datastore which would cause any problems.

Created another item and my missing item has reappeared.

Most odd!

What is the "pagination problem"? We use Tracker here extensively and I've not seen this...

On the Feature Requests tab I'm seeing duplicate items on page 2 (they already appeared on page 1).

In this video I have 15 feature requests. Note the completed item "Combined youtube/vimeo/bob video section"

I mark one of them as completed (the item titled "test").

Returning to the list, you can see the order of everything has changed (it seems randomly).
And now "Some kind of modal section" appears twice (last item on the first page, 4th item on the second page) whilst "Combined youtube/vimeo/bob video section" does not appear at all.

It's going to take about 45 minutes for vimeo to convert the video
