Approved items does not disappear. And also sometimes a user submits 3 items and I see 6 items in the DAB. But the list in the Pending Items Tab is correct.

That bottom section should not be there, there are 2 copies of the tabs showing. Try it on a different skin which has no alterations to it.

Do you see this with specific modules? I've not seen an issue with this on Jamroom.net or locally.

If you have pending enabled for both create and update, and a user UPDATES the item after creating it, but before you view it in the pending section, you'll get a CREATE pending item and UPDATE pending item.
