I keep getting Failed to initialize plugin: code after I upgraded

Run the integrity check:
ACP -> CORE -> SYSTEM CORE -> TOOLS -> INTEGRITY CHECK -> check all checkboxes -> START

Then reset the cache on your browser.

If the problem persists, try a different browser.
