Make sure the module is enabled, then enable it for all of your Quotas in the paypal quota config. Make sure you disable the FoxyCart module if you have that installed.

Then, when you create/modify an audio item you can set a buy-it-now price.

Working for me.
Make sure that the profile has his PP email set, otherwise the BuyNow button will not show with the item for sale.
If that is not the problem, please explain more. "Still not working" doesn't give us much of a clue ;-)
