Icons can be found in the modules/jrCore/img/icons_black and modules/jrCore/img/icons_white.

You may have icons in your skin directory that will override the core icons... skins/YOURSKIN/img/icons_black and skins/YOURSKIN/img/icons_white.

The reason you don't see the comment form is because you have it disabled in the Comment modules "View Options" tab....

/comment/admin/global/section=view+options
