Discussion "Follow" icon has disappeared for standard users

On Group discussions, we have set a Follow option and it used to work for all users. In the past week or so, standard users are reporting that they are not seeing a Follow icon on discussions. I can see one in my Admin user, but not in a standard user.

Has something changed in the system to affect this? I have made no changes to the permissions before this happened and the Follow icon is 'active' in Group Discussions. Since it was reported, I have tried changing
core item_action_buttons/ detail/m=jrGroupDiscuss
Group - from '(no group restrictions)' to 'logged in users'
and I changed the Quotas, which previously was unset, to "All Users". I cleared caches but the button is still not showing for standard users.
Any ideas?

Thanks for that advice. We do have legitimate members all over the world, so I am quite careful.

Unfortunately we have had what I assume is a spammer attack, with around 25 new sign ups, different user names all ending in Ruips, and all random email addresses. I have banned the most common IPs but they are not all the same. I have also tried to ban Ruips at the end of the user name - not sure I've done that correctly. Do you know if it should be %Ruips or just Ruips in the banned user name list?

About this:
I am trying to delete users and their profiles from the Pending User list, but although the pop-up text is that both will be deleted, it seems only the Profile is deleted and not the Account. Perhaps it's a cache issue, but the users are not deleted from the Pending list. When I go to Modify the user, it warns that no Profile is associated with it and the user will be unable to log in.

List of Pending Users shows only recently pending.

This is now showing all the pending users, but there seems to be a problem selecting the 'number of items per page', and the page jumper's page number.
No matter how many items "per page" I select on the page jumper, it only ever shows 12 per page.
When I scroll the page selector, it shows sequential numbers up to 99 and then the next page number is 275 (regardless of number of items per page that I select).

List of Pending Users shows only recently pending.

Looking on the Dashboard at Pending Users, it is showing only recent users who are still pending (those who joined the site up to about a month ago, but have not yet confirmed their emails).

If I go a few pages through the Data Browser of Users, there are a lot more users that are still pending, but don't appear in the Pending list. These are users who signed up anything from about a month ago to several years ago.
Is there a setting for number of Pending users shown, or some other option where I can see ALL the user accounts that are Pending?
